Woodland Hills Standard Application
Pennsylvania Teaching Certificate (teachers only)
Resume, Child Abuse, Criminal Check, FBI Criminal History, Arrest/Conviction Report Form PDE-6004 and Tuberculosis Test (all must be within the current year)
